Depending on experience of the selected candidate, the incumbent will be hired at the CSWI or CSWII level.
Under close supervision of the Care Coordination Leadership, provides social work services consisting of discharge planning, continuing care resources and clinical social work services, including crisis intervention, patient advocacy mandated reporting, informational and referral coordination of health and mental health services with community providers. The social worker is responsible for education of patients, families and/or significant others. Participates in related department functions, such as community and program education.
A Master's Degree from an accredited school of social work; or an equivalent combinationof education and experience; and knowledge and abilities essential to the successfulperformance of the duties assigned to the position.
Must be registered with the CA Board of Behavioral Sciences as an ASW intern.
Must possess a Associate Clinical Social Worker (ACSW) issued by the California Board of Behavioral Science Examiners.
Must complete CCS (California Children Services) paneling application within 30 days of hire.
Must have experience in working with diverse age groups from adolescent to geriatric populations.
Must complete all annual department competencies as required by Care Management Leadership.
Must be knowledgeable in all mandated reporting policies and practices.
Worked collaboratively with case managers in coordinating the after care needs of patients post discharge.
Familiar with case management principles and procedural requirements for the different health plans.
At least one to two (1 - 2) years of post MSW experience in the acute cares setting and/or related field.
EMR (Electronic Medical Record) - Epic documentation process experience.
Bilingual in both English & Spanish.
Must be able to work various hours, days, shifts, on-call and various locations based on the 24-hour Medical Center's business needs.
