UW Medical Center has an outstanding opportunity for a full-time Pharmacist 3. The Pharmacist 3 will be responsible for providing clinical pharmacy services in General Oncology at the Seattle Cancer Care Alliance (SCCA)/University of Washington Medical Center. This position will implement and monitor, with the health care team, patient medication therapy for achievement of desired therapeutic outcomes. The Pharmacist 3 will maintain accurate medication profiles for patients and utilize the system of Continuous Quality Improvement (CQI) to measure the effectiveness of pharmaceutical care. Participate in protocol development. S/he will participate in the education and training of health care students and providers and, perform the duties of a Pharmacist 2. The Pharmacist 3 will maintain patient confidentiality according to UW Medical Center standards and perform related duties as required.
Legal Requirement: Pharmacy degree (eligibility for State of Washington Pharmacist License)
Two years of experience in pharmacy practice.
Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
