We are currently seeking a Social Worker for our Care Management Department at Evanston Hospital. This is resource position and is not benefits eligible. Hours will be scheduled on an as needed basis. Weekday shift availability is essential.
Resource staff will be required to work a minimum of at least 2 (two) weekend shifts per month (8:30am - 5:00pm, Saturday and/or Sunday). Shifts may be assigned at any of our four hospitals (Evanston, Skokie, Highland Park, and Glenbrook).
Candidates will be required to attend initial trainings, which are assigned weekdays (Monday - Friday). Additional shifts are generally available periodically.
In this role, you will:
â¢Conduct and document assessment of patient and family needs, communicate care issues and develop appropriate plan of social work interventions
â¢Provide counseling and crisis intervention on an individual or group basis
â¢Patient advocacy including, but not limited to, education of patient and family rights, case management services and staff education regarding identification and reporting of abuse/neglect
â¢Discharge planning including, but not limited to, education on community resources and referrals while collaborating with a healthcare team
Our ideal candidate will have:
Master's degree from an accredited school of Social Work
Social Worker Licensing: LSW (Licensed Social Worker) or LCSW (Licensed Clinical Social Worker) required
Two years of postgraduate healthcare or mental health experience preferred
