Following established Home Health policies and procedures, and under the general supervision of the physical therapist, assists in performing tests, evaluations, and/or treatment procedures.
Assists in formulating an individualized treatment plan for each patient approved by a physical therapist based on evaluation and information gathered from Social Service, physician, the patient, and his family.
Schedules patients for treatment in accordance with physician, patient assessment and treatment plan.
Conducts treatment sessions, individually or in a group, according to treatment plan, under the general supervision of a licensed physical therapist.
Gathers biophysical, psychosocial, spiritual, cultural, and age appropriate information from patients, including information on the signs/symptoms of abuse or neglect. In conjunction with other disciplines, assists in developing a realistic comprehensive patient/family educational plan, using appropriate resources and methods of instruction and providing patient/family health teaching.
Maintains accurate and timely records of all treatment procedures, including progress notes, discharge summaries, and daily documentation, as approved by a licensed physical therapist.
Continually evaluates patient progress and makes necessary revisions to treatment plan in order to meet the patient's needs and treatment team goals.
Consults with physicians, family members, and other rehabilitation personnel to assist in coordinating the patient's total rehabilitation.
Provides education and training to patients and family members in order to maximize patient's independence at home and in the community.
Supports treatment plans being carried out by other disciplines as part of the treatment team goals. Participates in special committee work, as assigned.
Stays abreast of current trends and changes in rehabilitation through participation in inservices, seminars, workshops and professional organizations.
The above duties and responsibilities are performed in a manner that is attentive to the developmental and chronological age of the patient, when appropriate. Performs other duties as assigned.

Current OK PTA license
Valid driver's license
1 years experience as PTA
Requires effective oral and written communication skills to communicate with customers.
Requires good decision making and problem solving skills.
This position requires daily travel to patient homes and meeting locations, the ability to operate a vehicle in a safe and lawful manner is an essential function of the job. Additionally, access to a vehicle in good working condition required.
