This position is primarily responsible for implementing day-to-day case management interventions for identified Heart Failure members. Candidates must be bilingual Spanish/English. This is a Telecommute / Work From Home position and the schedule is Monday - Friday during normal business hours. If you are located in or near Miami, FL, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities:Making outbound calls to assess members' current health status Identifying gaps or barriers in treatment plans Providing patient education to assist with self-management Interacting with Medical Directors on challenging cases Coordinating care for members Making referrals to outside sources Coordinating services as needed (home health, DME, etc.) Educating members on disease processes Encouraging members to make healthy lifestyle changes This role is a telecommute position from the greater Miami, FL area. Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential.
Required Qualifications:Active, unrestricted RN license in the state of Florida 3+ years clinical experience (any setting) Bilingual skills (fluency in Spanish/English); speak, read and write Intermediate / proficient level of experience with MS Office applications Ability to handle multiple tasks and function in a high volume environment Must be willing / able to obtain CCM within 30 months of employment if you do not already have itMust live within a commutable distance of our Miami, FL office locationPreferred Qualifications:BSN Case management and / or disease management experienceCertified Case Manager (CCM) Experience working with heart failure patients Knowledge of Medicare guidelines and regulationsExperience working in Med Surg, Home Health, ICU, Cardiac ClinicIf the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
