The goal of the Field Care Advocate (FCA) role is to help individuals live their lives to the fullest by promoting recovery and resiliency via coordination and collaboration with multiple internal and external partners including consumers and their families/caregivers, medical, behavioral health and clinical network teams. Field Care Advocates work with complex and high-risk needs with a goal of engaging the consumer in the treatment process, decreasing their reliance on higher levels of care, helping them to access appropriate community services, and assisting them in improving community tenure. Field-based Behavioral Health Care Advocates are needed in the following areas: Alexandria, Greater New Orleans, Orleans Parish, Northshore, St. Tammany Parish, Livingston Parish, Thibodeaux, and Terrebonne Parish. Must be able to accommodate daily field-based travel within one of these regions. If you are located within a commutable distance of any of the above areas in Louisiana, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities:Oversee initial inpatient admission for psychiatric and chemical dependency patientsMake patient assessments and determine appropriate levels of care Administer benefits, review treatment plans, and coordinate transitions between various areas of careObtain information from providers on outpatient requests for treatment You'll find the pace fast and the challenges ongoing. We'll expect you to achieve and document measurable results. You'll also need to think and act quickly while working with a diverse member population.
Required Qualifications: Licensed Master's degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2 or more years of experience in behavioral health Licenses must be active and unrestricted in your state of residence2+ years experience in a related mental health settingComputer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applicationsAccess to high speed internet from homeReliable transportation and the ability to travel as needed within the designated service delivery area (up to 50%)Preferred Qualifications:Experience in an inpatient setting Dual diagnosis experience with mental health and substance abuse Experience in a community mental health settingExperience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients Previous experience in a managed care environmentExperience with the Medicaid/Medicare populationDiscover a diversified health services and innovation company where data, technology, people and action combine to make great things happen every day – and where every day is another opportunity to do your life's best work.(sm) *All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.
