The Infectious Disease Pharmacy Specialist (IDPS) will develop and enforce hospital supported programs designed to ensure safe, effective and cost-efficient use of anti-infectives across the PHC system. The IDPS will coordinate and maintain a program in line with the goals and directives of the PHC Antimicrobial Stewardship Committee and uphold Joint Commission Regulations.
Develops, enforces and maintains policies and procedures that promote and enhance patient outcomes through safe and cost-efficient medication therapy for patients receiving anti-infectives.
Coordinates program designed to minimize medication errors, adverse drug reactions and medication misuse with regards to medication preparations in infections disease through improved reporting, analysis and timely follow-up.
Coordinates and maintains the antimicrobial stewardship program, including:
Overseeing daily prospective auditing of anti-infectives
Assisting in development and maintenance of an antimicrobial formulary approved by the P&T committee
Working with the infections disease service to evaluate appropriate use of restricted anti-infectives
Developing guidelines, policies, and other monitoring and intervention strategies for anti-infectives use of streamlining prophylactic, empiric, and definitive antimicrobial therapy
Compiling data and metrics regarding antimicrobial utilization
Working with the Antimicrobial Stewardship Committee to develop and monitor compliance with medication guidelines
Reporting data and findings to the Antimicrobial Stewardship Committee, Infection Control and Pharmacy & Therapeutics Committees
Coordinating, facilitating and promoted education, training, competency assessment and performance improvement of pharmacists and other healthcare professionals regarding the appropriate antimicrobials.
Pharmacist License in the state of Wisconsin Bachelor's in Pharmaceutical Sciences Required Preferred Education level – Doctorate Basic Life Support (BLS) Required Valid WI Driver's license Post graduate training (Pharmacy Practice Residency and/or Specialty Residency in Infectious Disease) or equivalent experience He or she must possess a broad scope of pharmacotherapy knowledge and skill for providing direct patient care as well as providing complex, in-depth clinical services.
**Effective January 1, 2017, The Joint Commission released a new Medication Management (MM) standard (MM.09.01.01) for hospitals, critical access hospitals, and nursing care centers. The IDSP will develop programs to adhere with Joint Commission regulations and provide surveillance and education of the medical staff
