The LCSW will provide services to individuals, families and groups to assist with improvement of social functioning and maximize strengths; interacts and interfaces with the community in diverse roles of advocacy and overall planning for services utilization; member of the interdisciplinary team and contributes to the comprehensive health care services.
ESSENTIAL DUTIES AND RESPONSIBILITIES(included but not limited to the following);
Performs Bio-psychosocial assessments.
Provides information and referrals for counseling and service acquisition as appropriate to the assessed need.
Provides consultation services.
Initiate treatment interventions; e.g., individual, family and group counseling and crisis intervention.
Documents client interaction and referrals.
Performs patient education.
Make home visits as needed.
Participates in data collection and compilation.
Completes all necessary statistical reports and is accountable for accuracy.
Develops and implements policies and procedures.
