Position Summary: We have an exciting opportunity to join our team as a Social Worker.
In this role, the successful candidate The Social Worker treats patients as part of the NYU Langone Hospital- Brooklyn DSRIP Behavioral Health Integration Program. The goal is to expand the delivery of behavioral health services at primary care centers within the Family Health Centers of NYU Langone Network. The Social Worker will be based at one or more sites in Brooklyn and will have close collaboration with the primary care staff and be part of a medical team. The main function will be provide a brief model of psychotherapy (generally up to 20 visits) to patients who screen positive for mild to moderate mental health conditions in a medical setting; work closely and provide feedback to the referring PCP; develop and carry out treatment plans; refer patients out to more intensive mental health of substance use services when necessary.
Participate in the site's team meetings and daily huddles, with a focus on care coordination, educating medical staff about the role of the social worker and the importance mental health assessment and treatment.
Provide clinical feedback on patient progress to the referring PCP
Provide confirmation calls and outreach calls as appropriate
Document psychotherapy visits/ progress notes in the electronic medical record within 72 hours.
Develop and carry out a treatment plan in the electronic medical record based on the presenting behavioral health issues
Provide brief psychotherapy, preferably using an evidence based treatment (i.e., Problem Solving Therapy/collaborative care model), up to 20 sessions.
Conduct biopsychosocial assessment focusing on mental health and substance use evaluation
Receive medical patients from the PCP when they screen positive for mental health conditions ('warm hand-off' referral)
Minimum Qualifications: To qualify you must have a LCSW/LMSW in Social Work, license current and in good standing Two evening shifts are required (11am - 7pm shift). Bi-lingual English/Spanish required
Required Licenses: Lic Clinical Social Worker
Preferred Qualifications: - Bilingual English/Spanish - Comfortable working in a medical setting - Familiar with use of electronic medical records - Familiar with medical conditions and terminology
