Join us and help people live healthier lives while doing your life's best work.(sm) Primary Responsibilities:Provide physical therapy services to patient according to physician's plan of care. This may include, but will not be limited to assessing and evaluating therapeutic / rehabilitative / functional status, participation in the development of the total plan of care and evaluates home environment Initiates physical therapy program and instructs other personnel and / or family caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences Prepares and submits clinical and progress summaries in a timely manner Participates in discharge planning for patient Supervises physical therapy assistants according to organization policy and state regulations, treat and re-evaluation patient no less than every 7th day of treatment or 21 days whichever occurs first Maintains contact / communication with other personnel involved in the patient's care to promote coordinated, efficient care Maintain clinical competency in physical therapy practice and theory Will participate in Agency on-call coverage as outlined in Agency policy
Required Qualifications: Graduate Degree from an Accredited Physical Therapy Program Possess an active and unrestricted Nevada Physical Therapist license 2+ years of experience as a Physical Therapist CPR certification or obtain within 30 days of hire Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area Preferred Qualifications: 1 year of Home Health experience Experience with Electronic Medical Records Systems Experience with OASIS documentation Experience in a home health setting Weekend availability Careers with Optum.
