If you are located in the greater Jacksonville, FL area, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities: Provide a complete continuum of quality care through close communication with members via in-person or on-phone interactionSupport members with condition education, medication reviews and connections to resources.Assist members with the transition from a care facility back to their homeMaking outbound calls to assess members' current health statusWorking with the complex care needsIdentifying gaps or barriers in treatment plans Providing patient education to assist with self-managementInteracting with Medical Directors on challenging caseMaking referrals to outside sourcesCoordinating services as needed (home health, DME, etc.)Educating members on disease processesEncouraging members to make healthy lifestyle changesDocumenting and tracking findingsVisit members in their homes and/or other settings, including community centers, hospitals or providers' officesThis role involves approximately 25% travel in Jacksonville, FL and you'll need to be comfortable dealing with a full range of in-home and facility-based environments. Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential.
Required Qualifications: Current, unrestricted RN license in the state of FloridaBilingual Spanish/EnglishAccess to reliable transportation2+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management Proficiency with Microsoft Office Suite, with the ability to navigate a Windows environmentComputer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applicationsCandidates are required to obtain a CCM within 30 months of employment if you do not already have oneAbility to travel in the greater Jacksonville, FL area approximately 25% of the timePreferred Qualifications: Case management experienceCertification in Case ManagementBSNHome care/field based case managementExperience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needsExperience in Home & Community based, Hospice, Home Health or Long Term Care services deliveryIf the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
