Counselor – Central FL Region –Community Care – Courtland
Florida Hospital Courtland seeks to hire a Counselor – Central FL Region who will embrace our mission to extend the healing ministry of Christ.
Established in 1908, Florida Hospital is one of the largest not-for-profit healthcare systems in the country, caring for more than a million patients each year. The Courtland Office Plaza is conveniently located a few miles outside downtown Orlando and is home to over 700 employees that support our hospital system including the Florida Hospital Home Care Service, Management Information Systems, Nursing Informatics and Florida Hospital Healthcare System.
Monday – Friday; 8a – 5p
The Community Care Counselor provides patient behavioral assessments, counseling services, eliminates care gaps and works with the core Community Care team and partners such as physicians, pharmacies, home health care, hospice, skilled nursing facilities, inpatient rehabilitation facilities, and universities. This program is comprised of high-user patients in our community. Adheres to all rules and regulations of applicable local, state and federal agencies and regulatory bodies and Florida Hospital policies and procedures. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
Professional knowledge of mind-body medicine and current research within field
Proficiency in Word, Excel, Outlook, PowerPoint and Access
Ability to support departmental and organizational mission
Minimum of three years of counseling experience
Minimum of two years of experience utilizing guided imagery both with individuals and with groups
Licensure, Certification, or Registration Required:
Current valid State of Florida Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC)
Current valid State of Florida driver's license, class E and driving record in accordance with Florida Hospital policy
Current valid auto insurance

Communication: Ensures timely communication with patients, significant others and other healthcare providers through professional collaboration
Utilizes effective oral and written communication skills, maintains required level of confidentiality and interacts effectively with employees at all levels.
Discusses referrals with physician when appropriate.
Participates in community education and outreach programs related to Florida Hospital Community Care (FHCC).
Collaborates with other healthcare providers to deliver patient-centered care coordination in a manner consistent with safe, efficient and cost-effective resource utilization.
Coordinates with other disciplines and community resources.
Discusses progress of patients with other members of FHCC team, as appropriate.
Works collaboratively with the FHCC team to integrate service delivery system for co- morbidity/co-occurring treatment.
Works in collaboration with FHCC, subsidiaries, and external collaborating agencies/groups to develop a system of continuity of care.
Plans, implements and documents patient visits.
Documents on chart timely and appropriately
Develops educational materials as necessary for use with patients and families.
Assists with community outreach educational programs related to care coordination.
Assists with development and distribution of educational materials as necessary for use in community programs.
Assists with the assessment of needs, development and presentation of community medical and psycho-educational trainings.
Collaborates with FH Education Department on accreditation of trainings for professional continuing educational credits and for the marketing, registration and presentation of psycho-educational trainings.
Performance: Provides patient assessment for medical and behavioral needs.
Physical, psychological and emotional outcomes are individualized and identified
Formulates a plan of care based on assessment, diagnosis, age, Health Risk Assessment and patient profile.
Systematic and pertinent collection of data for each patient.
The plan of care is consistent with safe, effective patient care and age.
The urgency of medical, psychological, social and emotional needs is determined, and the course of action is formulated to attain expected outcomes.
Collaborates a plan of care based on assessment, diagnosis, age, Health Risk Assessment and patient profile.
Interventions are based on patient need and consideration of patient and/or other.
Evaluates and modifies plan of care based on patient responses and attainment of expected outcomes.
The patient's response to the intervention is regularly evaluated to determine progress toward resolution of needs.
Adheres to established standards of practice including activities and behavior that characterize professional status.
Is accountable for his/her actions.
Functions as core member of FHCC team.
Provides leadership for other team members.
Organizes program materials, content and schedule.
Assists FHCC team in efforts to promote care coordination
Evaluates comprehension of information presented.
Supervises student interns.
Other duties as assigned.
