At Optum, our Wellness Staff play a pivotal role in our mission to help people live healthier lives. This position involves working as a member of a multi-professional team promoting and supporting people to develop healthier behaviours and lifestyles. Delivering telephonic Coaching, support using a range of evidence based behavior change techniques you will support clients to make long-term health behaviour change including weight management, physical activity, smoking and alcohol. This role requires the individual to work remotely from their home using outbound telephone calls (equipment provided) as a means of liaising with your client base. There will be flexible working requirements the service offers appointments from 8am – 8pm. You will also be required to travel to meetings. Primary Responsibilities:Undertake assessments of individuals in relation to their readiness, capability, opportunities and motivation to change using the Optum Assessment and COM - B (with training and guidance): Responding to referrals and utilising evidence based behaviour change techniques support the population to adopt healthier lifestyles Undertake assessments including (but not limited to) IPAQ, AUDIT - C, BMI, WEBWMS Support individuals to develop their knowledge about their own health and well - being Provide telephonic Coaching support to individuals about health and well - being (specifically weight management, physical activity, smoking and alcohol) Signpost individuals to other agencies / organisations for information, support and resources where appropriate To encourage and motivate people, using creative ways of addressing negative behaviours Enable clients to make choices and develop an 'action plan' using coaching techniques to identify SMART goals to change behaviour and sustain those changes, leading to more healthy choices and actions by interpreting local and national strategy Support individual's 'action plan' over an agreed timescale and to review and revise 'action plans' with individuals as appropriate with appropriate level of follow - up Alert line manager to any concerns raised by client in line with safeguarding policies Adhere to organisational processes, policies and procedures including clinical governance and confidentiality To be respectful and non - judgmental and encourage clients in choices and actions that are acceptable and achievable to them, while being aware of barriers to adopting a healthier lifestyle e.g. cultural considerations, affordability, accessibility and life circumstance Manage all calls in a controlled and professional manner, maintaining a high level of confidentiality in all aspects of work including client records and conversations, whilst building a relationship and supporting clients to change their behaviours To maintain accurate information systems of records and activities using a Case Management System (Database) Maintain and manage own diary and calendar, booking appointments with clients Provide excellent customer service To attend and support local wellbeing events and to represent the service Attend meetings and training sessions as requested by line manager to develop skills
Required Qualifications:Educated to GCSE Standard or Equivalent with A-C grades in English and Maths Experience with Microsoft Office packages: Word, Excel, PowerPoint and Databases Experience of working with people in a coaching environment A recognised coaching qualification or demonstrable experience of supporting people using coaching models within a behaviour change setting Experience working with behaviour change theories (e.g. ASM, Stages of Change) / coaching models (e.g. GROW) Have an interest in behavioural change and working with adults to help change their lifestyle Experience in coaching individuals to make changes in their behaviour using evidence based behaviour change techniques Proven experience of showing a willingness to help others and have an interest in the subject matter area.
