Join us and help people live healthier lives while doing your life's best work.(sm) As a dedicated Housing Specialist(s), you shall be responsible for helping to identify, secure, and maintain community-based housing for MLTSS/Medicaid health plan Members and for developing, articulating, and implementing a broader housing strategy to expand housing availability/options. The Housing Specialist(s) shall act as the health plan housing expert(s)/resource(s), providing education and assistance to all relevant staff (care managers and others) regarding supportive housing services and related issues for MLTSS and Medicaid Members. The Housing Specialist shall act as a liaison with state Medicaid (DMAHS) staff, or its designee, to receive training and capacity building assistance. Primary Responsibilities:Clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating) Case management and coordination of care Takes lead role in setting direction and participating in or developing new programs Sets team direction, resolves problems and provides guidance to members of the team Ensures team meets established performance metrics and performance guarantees Adapts departmental plans and priorities to address business and operational challenges The Housing Specialist(s) will maintain records and reports of all activities
Required Qualifications:3 years full-time experience in assisting vulnerable populations (e.g. homeless, elderly, people with disabilities, etc.) to secure accessible, affordable housing. Must be familiar with relevant public and private housing resources and stakeholders, including but not limited to HUD subsidized housing, all Department of Community Affairs (DCA), New Jersey Housing and Mortgage Finance Agency (NJ HMFA) housing program voucher programs, public housing authorities, realtors, and online housing locator resources. 1 year of clinical or case management experience 1 year of experience in long-term care, home health, hospice, public health or assisted living Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, Internet research, use of online calendars and other software applicationPreferred Qualifications:BSN A background in managed care Case Management experience Certified Case Manager (CCM) Experience / exposure with discharge planning Strong organizational skills and multitasking abilities will be keys to success Microsoft Excel proficiencyCareers at UnitedHealthcare Community & State.
