Nurse Practitioner works autonomously to provide direct patient care and manage a designated group of patients in a contracted skilled / subacute / rehab / group homes / independent living facility. Performs aspects of care including history and physicals, wellness exams and screenings, assessments, coding and documentations, and recommendations to clinical team. Nurse Practitioner works in collaboration with a physician and provide cross coverage with the Lifestyle Center Program. Primary Responsibilities: Assess, examine and establish differential diagnoses from assessments conducted within the clinical setting for patients admitted to a contracted facility Develop and implement preventive measures, treatment plans and recommendations to clinical team Work collaboratively with physician and clinical team to acquire direction and advice on cases or patient issues Identify and direct complex cases to physician care team for handling and care planning Develop and implement treatment plans in conjunction with the physician and other members of the care team Document treatment plan and collaborative physician guidance in the medical record Actively communicate with outpatient primary care and specialty care providers when needed Order appropriate laboratory tests and perform appropriate screening tests and diagnostic procedures Review medications for polypharmacy compliance and educate patients on medications Collaborative review of chronic preventative measures for seniors for quality review Provide guidance to nursing and therapy staff regarding treatment plans and care coordination Coordinate care with patients and care team
Required Qualifications:Successful completion Advanced Practice Nurse degree or Physician Assistant Degree from an accredited university Ability to obtain and maintain an active, unrestricted NP / PA Nevada License in area of practice Nurse Practitioner Certification as an Adult, Family or Geriatric Nurse Practitioner from either ANCC or AANP required, or certified Physician Assistant or ability to obtain Minimum two years of successful clinical experience in a primary or inpatient care setting Ability to obtain DEA licensure Knowledge of professional medical practice to provide care, formulate a differential diagnosis and evaluate patient Knowledge of screening and diagnostic equipment and instruments to administer patient care Technical knowledge in the use of computer programs for medical records or internet searches for medical research Skill in applying and modifying the principles, methods and techniques of medicine Demonstrated excellent communication skills with patients, family members, care team members and management staff Ability to make care decisions based on best medical practices guidelines without direct supervision or guidance by a physician Has basic technical proficiency in the use of computer programs for medical records or internet search for clinical practiceOptumHealth is part of the family of companies that make UnitedHealth Group one of the leaders across most major segments of the US health care system.
