Patient placement coordinator tracks, performs and enforces all activities which involve the placement and movement of patients and helps to monitor efficient care transitions. This includes following up on ED patients admitted to the hospital through the various portals of entry and on patients who have been discharged. The patient placement position assigns patients to beds based on collaboration with nursing leaders and physicians regarding care requirements and for maximizing the use of the hospital bed resources appropriate for care. The coordinator will work collaboratively with the Emergency Department, Nursing, Case Management and Environmental Services in regard to utilization management and review of beds. The coordinator will communicate anticipated staffing needs to nursing based on hospital census. POSITION RESPONSIBILITIES: 1. Identify and assign beds for all admissions from various access points to include: the Emergency Department, Surgery, Cardiac Cath, pre-admission, and same day admission, direct admits, transfer from ICU, as well as interfacility hospital direct admissions. 2. Helps in expediting discharges through communication, follow up, and having a hands-on approach. This position is a highly visible position which entails rounding on the units/ED continually throughout the day. 3. Follows up in the Emergency Department on admitted patients to ensure they are moved swiftly onto the floors. 4. Monitors bed board of patients and notifies supervisor and nursing staff of changes in bed status at the start of the shift and throughout the day. Works with the
