UnitedHealthcare (TeamMD) is recruiting for an experienced Licensed Mental Health Therapist to work as part of an interdisciplinary team with myConnections to provide integrated health and social services to individuals who are homeless or at risk of homelessness and suffering from substance use disorder. The greatest barriers to better health for many people enrolled in Medicaid are not clinical issues, but rather social or financial barriers, such as lacking access to safe and affordable housing or having no way to travel to a job or needed services. MyConnections integrates, organizes and distributes support services for consumers and provides people with greater access to safe housing, transportation, education and job training, which are essential to good health. The Licensed Mental Health Therapist will be embedded within a transitional housing complex and will work closely with an interdisciplinary team to assess individual needs, help individuals transition safely from hospital or homeless shelter, provide post-acute care, and coordinate referrals and services with local primary care and community based organizations. Dialectical Behavioral Therapy is an innovative, evidence-based cognitive behavioral treatment program. Research demonstrates that it is not only recognized as the "gold standard" psychological treatment for chronically suicidal individuals diagnosed with borderline personality disorder (BPD), but that it is effective in treating other disorders such as substance dependence, depression, post-traumatic stress disorder (PTSD), and eating disorders. If you are located in Phoenix, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities: In consultation with other professionals, determines the rehabilitation approach that will best meet the needs of the patient Conduct individual and/or group therapy sessions Assess patient's progress and modifies or extends treatment program accordingly Provides therapeutic intervention with patient and family including individual, marital, parental, family and group therapy sessions Provide clients with new skills to manage painful emotions Help clients decrease conflict in relationships Give clients techniques that allow them to communicate with others in an assertive way that maintains self-respect and strengthens relationships Interview patients and family to obtain information about the home environment Document current, accurate observations of care to the patient in appropriate records Maintains confidentiality of all patients Will integrate values of justice, respect, compassion, excellence and stewardship into provision of services Will work with diverse care teams in a variety of settings This position requires effective time management and communication skills, ability to work independently, and to be detail oriented This position requires an individual who is dependable and reliable
Required Qualifications:Master's in Counseling ( LMHC, LPCC), Psychology (PhD) or social work (LMSW, LCSW) 5+ years of clinical experience in mental health and substance use disorder 3+ years of psychotherapy in a community settingExperience in working with diverse vulnerable populations in a therapeutic settingWilling to work a mix of day and evening hoursAccess to reliable transportation that will enable you to travel to client and / or patient sites within a designated area in Phoenix, AZAbility to travel to visit members within a designated area.
