As a Field-Based and Telephonic RN Case Manager, you'll wear many hats and work in a variety of environments. As a Pediatric Field Based and Telephonic Case Manager, you'll interact with medically complex children. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Assisting Medically Complex Children and providing family support with expert symptom management, and care coordination. If you are located near Orlando, Florida, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities: Visit members in their homes and/or other settings, including community centers, hospitals or providers' officesProvide a complete continuum of quality care through close communication with members via in-person or on-phone interactionSupport members with condition education, medication reviews and plan of care developmentAssist members with the transition from a care facility back to their homeMaking outbound calls to assess members' current health statusWorking with the complex care needsIdentifying gaps or barriers in treatment plans Interacting with Medical Directors on challenging caseMaking referrals to outside sourcesCoordinating services as needed (home health, DME, etc.)Educating members on disease processesDocumenting and tracking findingsProvide monthly updates to Agency for Health Care AdministrationThis role involves approximately 25% travel in the greater Orlando, FL area and you'll need to be comfortable dealing with a full range of in-home and facility-based environments.
Required Qualifications: Current, unrestricted RN license in the state of FloridaAccess to reliable transportation2+ years clinical experience in NICU and/or Pediatrics, hospital, acute care, home health/hospice, direct care or case management Experience working with children with chronic illness and complex care needsProficiency with Microsoft Office Suite, with the ability to navigate a Windows environment, excel spreadsheet, experience with email, internet research, use of online calendars and other software applicationsCandidates must obtain a CCM within 30 months of employment if you do not already have oneMust be able to travel in the greater Orlando, FL area at least 25% of the timePreferred Qualifications: BSNCase management experienceCertification in Case ManagementHome care/field based case managementExperience working with the needs of vulnerable populations (pediatric members) who have chronic or complex bio-psychosocial needsBilingual Spanish / EnglishExperience working with pediatrics/NICU as a specialtyExperience in Home & Community based, Hospice, Home Health or Long Term Care services deliveryIf the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
