Reach for more and discover your life's best work.(sm) Provides direct intervention, assessment, and consultation of behavioral health issues impacting chronically ill and high risk patients in a variety of environments including private homes, assisted living facilities, nursing homes, skilled nursing facilities, and/or hospital settings. The LCSW works in collaboration with a multidisciplinary team to identify and access services to close care gaps and improve quality of life. Primary Responsibilities:Provides direct short - term evidence-based therapies for stabilization of individuals with complex medical and behavioral health issues Provide support to medical staff to include consultation, assessment, intervention, and education / training of behavioral health needs and services Identify, refer, and coordinate available community resources to address non - medical needs and ensure patient access to services Clearly summarize pertinent clinical information in the patient record Share clinical expertise with medical staff to ensure continuity and quality of care Assist the multidisciplinary team in identifying and implementing effective communication methods to engage patients with behavioral health issues Participate in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of service delivery
Required Qualifications:Master's Degree from an accredited Social Work (MSW) program Clinical Social Work license in the state of hire (LCSW) 3+ years' experience working with individuals with severe mental illness, complex social/behavioral needs, and/or challenging living situations Ability to work with diverse care teams in a variety of settings Effective time management and communication skills Knowledge of and experience with, behavioral health services, psychotropic medications, and therapeutic interventions to manage symptomsAccess to reliable transportation that will enable you to travel to client and/or patient sites within a designated areaAbility to travel up to 75% for face to face meeting with members in Des Moines, IA and the surrounding area.
