We are currently seeking a Social Worker for our Psychiatry and Behavior Science department located in Skokie, IL. This is a full-time position and is benefits eligible. Hours will rotate Monday thru Friday between 10:00AM - 7:00PM, in addition to rotating weekends.
In this role, you will:
Conduct and document assessment of patient and family needs, communicate care issues, and develop appropriate plan
Provide counseling and crisis intervention on an individual or group basis
Patient advocacy including but not limited to education of patient and family rights, case management services, and staff education regarding identification and reporting of abuse/neglect
Discharge planning including but not limited to education on community resources and referrals while collaborating with the health care team
Our ideal candidate will have:
LCSW license required
LCSW license for at least 2 years preferred
Master’s degree from an accredited school of social work required
Two years post-graduate experience preferred; healthcare and/or mental health experience preferred
Outpatient experience in clinical setting preferred
