CarePartners seeks a full time Hospice Physician in Asheville, NC This position is responsible for the following:
Summary:The Hospice and Palliative Care Physician is a member of the hospice interdisciplinary team and/or the palliative care consultation team. The primary role of the physician is to provide direct patient care as well as medical consultation to the interdisciplinary team. The physician will promote hospice and palliative medicine throughout Mission Health System / CarePartners and the community, including both formal and informal physician education. Position requirements are a Doctor of Medicine or Osteopathy from an accredited medical school and current North Carolina medical license.
Assesses patient’s medical and functional status by obtaining relevant health and medical history, performing appropriate physical exam based on age, history, and diagnosis, identifying health and medical risk factors, and assessing response to interventions.
Collaborates with the other medical staff, including physicians and nurse practitioners, the interdisciplinary care team, and administration in the provision of medical care.
Evaluates patient to determine appropriateness for admission following the service line specific criteria.
Reviews revocations, discharges, and recertification of terminal prognosis.
Provides expertise in pain and symptom management to the interdisciplinary team.
Initiates, monitors, and modifies individualized treatment plan.
Prescribes/orders appropriate pharmacologic and non-pharmacologic interventions.
Provides direct participant care services as needed
Participates in call and weekend rotation to ensure 24 hour coverage.
Prescribes appropriate pharmacologic and non-pharmacologic interventions.
Promotes best practices in hospice care and facilitates access to appropriate services.
Board certification in Hospice /Palliative medicine
Demonstrated clinical excellence in both medicine and communication skills
