Apply today! ****Must be available to work the following schedule: Monday - Friday, 11:30am – 8pm (some Saturdays) 40 hour work week. **** Office position Primary Responsibilities:Make outbound calls to assess members' current health status Identify gaps or barriers in treatment plans Provide patient education to assist with self-management Make referrals to outside sources Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels This is high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.
Required Qualifications: Current, unrestricted RN license in the state of New York3+ years of experience in a hospital, acute care or direct care settingMust be able to type and have the ability to navigate a Windows based environmentLive within a commutable distance (50 miles or 50 minutes) of Tonawanda, NY Ability to work 11:30AM - 8PM Monday - Friday (some Saturdays) onsite in our Tonawanda officePreferred Qualifications:BSNA background in managed careCase management experienceCertified Case Manager (CCM)Experience or exposure to discharge planning Experience in utilization review, concurrent review or risk managementPrevious experience in a telephonic roleAnswer the call to use your diverse knowledge and experience to make health care work better for our patients.
