Erie Family Health Center is looking for an outgoing, responsible and attentive individual like you to join our Patient Access Center! The Telephone Triage Nurse is responsible for expanding Erie’s ability to provide comprehensive, high quality care by providing assessment, education and treatment of individual patients over the phone.
Respond to incoming calls from patients with emergent and urgent concerns or questions regarding their health
Utilize sound clinical judgment, the nursing process, and decision support tools (protocols or guidelines) to assist patients.
Performs a systematic, comprehensive interview of the caller/patient to determine the urgency of the patient’s problem
Makes referrals to the appropriate level of care within an appropriate period of time
Schedules same-day appointments or follow-up appointments with the patient’s primary care provider or alternative health care provider as necessary
Accurately documents data for all calls including, but not limited to, patient’s demographic information, chief complaint, recommendations for home treatment, and referrals to primary care provider or alternative health care provider
Graduate of an accredited school of Nursing, current Illinois Registered Nurse License
2-3 years in critical care or comparable experience preferred
Knowledge in current concepts of medical-surgical, pediatric, obstetric/gynecology, behavioral health, and pharmacology.
Strong interpersonal and written communication skills.
Ability to communicate effectively in both English and Spanish (May be required to assist or communicate with non-English/Spanish only speaking patients and/or guests) required
