This position addresses goal management and progress tailored to patient health needs; provides psychosocial assessments, discharge planning, patient consultation regarding illness, diagnosis, treatment, and/or life situations. This position performs educational activities on health topics to CCHC patients; is responsible for assisting with projects that promote and advocate overall health and well being, including prevention and management of chronic diseases, nutrition, physical activity, prenatal care, etc. In addition, performs other duties as required. These functions are performed in accordance with the clinic’s philosophy, policies, procedures and standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides psychosocial assessments of patients to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
Provides education to patients around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.
Plans, develops, and facilitates educational activities for patients including but not limited to presentations, workshops, goal-setting and management, addressing techniques for health improvement, trouble areas, etc.
Develops materials and facilitates individual health sessions, group classes, and/or educational workshops regarding nutrition, exercise, chronic diseases, and other health topics.
Maintains working knowledge of and liaison with community agencies and resources.
Knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.
Documents assessments, plans, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.
Maintains records and statistics in accordance with department, clinic, and/or health system policies. Attends and participates in staff, committee, department, and other administrative meetings.
Participates in department, hospital, medical center, and/or health system continuous quality improvement efforts.
Participates in regular supervisory conferences; keeps current with social work and health care developments and seek to increase further enhancement of job related knowledge.
Provides in-service trainings for CCHC staff.
Researches and utilizes fact-based, non-biased information to inform educational activities.
Conducts evaluation activities for program effectiveness and efficiency.
Participates in case conferences and community agency meetings to improve service delivery.
May participate in community events including health fairs, street outreach, and other special events to promote clinical linkages, as needed.
Keeps records and performs data entry duties as required.
Attends trainings and conferences as needed.
Assists in formulation, planning, and implementation of policies, procedures and programs.
Develops and participates in the spread of new programs as needed.
Carries out other duties as assigned.
EDUCATION AND EXPERIENCE
Master's degree in social work from an accredited University.
Bilingual in Spanish or Armenian
Two years recent post master's experience in a clinical setting preferred.
Preferred background in public health, health science, or health education-related field.
One or more year’s previous experience in individual health counseling/group/class training/teaching.
Microsoft Office/computer experience required.

Must be detail oriented.
Must have reliable transportation.
Requires a full range of body motion including handling and lifting, manual finger dexterity and eye-hand coordination. Involves standing and walking. Requires normal visual acuity and hearing.
