We are now hiring for a full-time Social Worker/ Counselor 1 to join our Calumet team servicing Altgeld Gardens/Lowden/Scattered Sites!
ESSENTIAL JOB FUNCTIONS:
Provides Quality of Life clinical and crisis intervention services.
Completion of the mental health/substance abuse assessments, short-term individual counseling, treatment planning, psycho-educational groups and referrals for long-term intervention services.
Provides ongoing outreach and engagement to build relationships with clients in need of clinical and wellness services and inform them of available resources.
Completion of CHA standardized mental health assessment to understand presenting problem/situation, history, level of functioning, suicidal/homicidal ideation, substance abuse, etc.
Facilitate psycho-educational workshops and group counseling services.
Provides individual counseling services to meet client needs.
Coordinate community meetings and other events around violence prevention efforts.
Handles emergency and crisis situations with clients.
Does comprehensive, in-depth assessments for a variety of program services. Does intake as assigned.
Provides a full range of case management, counseling, advocacy educational, and related services.
Learns and utilizes agency and community resources.
Assesses community conditions affecting clients to identify resource gaps and problems.
Recommends financial assistance for clients.
Provides group services as indicated by program need.
May be responsible for meeting contractual requirements such as 24-hour coverage.
Represents and interprets programs of agency in community.
May serve on regional or agency-wide committees.
Meets agency expectation for quantity of direct service.
Complies with agency recordkeeping expectations.
Other duties as assigned by supervisor.
Works effectively with diverse staff and service population.
Master’s Degree in Social Work or Counseling from an accredited college or university required.
LSW or LCP required or be eligible to sit for these exams and obtain licensure within 6 months.
Ability to response to emergency and crisis situations with clients and/or staff.
Must be proficient in collaborating with community partners.
Must have excellent verbal and communication skills.
Requires mobility to work with a community-based caseload.
Must have access to a vehicle, valid driver’s license and proof of insurance to perform essential job functions.
Ability to interact appropriately with clients and staff and with diverse individuals and groups.
Ability to maintain assigned community and home-based services.
Ability to work effectively with diverse individuals and groups.
