Children’s Home + Aid’s Foster Care services provide safe, secure, and nurturing homes for children who have been separated from the care of their biological families. Twenty-four hours a day, 365 days a year, in communities all across Illinois, we give children a place to belong and the support they need to love, to learn, to heal, to dream, and to reach their potential.
This position provides nurse consultation services for youth in foster care who have identified medical conditions. While not providing direct clinical services, the Nurse provides consultation, training and advocacy for medically fragile children.
This position requires approximately 15 - 20 consultative hours per month. Most of the work is completed in the foster homes, which are scattered across Cook County. Visits often take place in the late afternoon/evening or on weekends in order to accommodate working parents and school schedules. The Registered Nurse is an independent contractor.
Monitor medical treatment plans for assigned youth.
Oversee medical appointments as needed.
Advocate for treatment on behalf of assigned youth.
Provide consultation and education to foster parents, biological parents and staff at least quarterly via in person contacts in the foster home.
Participate in quarterly file reviews and complete monthly case notes documenting the need for continuum of care.
Current State of Illinois Professional Nurse’s Registration or a PhD in good standing required.
Bachelor’s degree from an accredited university required.
A minimum of 2 years of professional nursing/medical experience required.
Valid driver’s license, auto insurance and reliable transportation required.
Sound judgment in a variety of medical situations required.
Ability to maintain strong working relationships with clients, agency personnel, and staff members of other social service and medical agencies.
Ability to work successfully as a healthcare professional in a multi-disciplinary setting.
