Home Health Care Provider, Integrated Healthcare Delivery System
Empath Health is a nonprofit integrated network of care that provides expert, life-changing health care to those with chronic and advanced illnesses. Empath Health includes hospice, palliative home health and physician services, independent living assistance and day care for seniors, HIV services and advance care planning. Suncoast PACE (Program of All-Inclusive Care for the Elderly) is a nonprofit provider of comprehensive health care and support services to Pinellas County seniors with chronic health conditions. As a member of Empath Health, our focus is on helping participants remain healthy and independent while living in their own homes.
We are currently seeking an experienced and compassionate physician to join the growing PACE family as the second physician. The primary care physician will provide primary care and continuous medical coverage, directly provides services to Suncoast PACE program participants.

OTHER SKILLS AND ABILITIES •Thorough knowledge of current concepts, theories and practices related to home and community-based care for the elderly and disabled adults. •Working knowledge of health care delivery and financing systems, including Medical, Medicare, waiver programs, prospective and capitated systems, public health programs and HMO/Managed Care. •Successful experience with MRA coding (desireable). •Working knowledge of the PACE regulations desired. •Thorough knowledge of physical, mental and social needs of frail older adults. •Effective oral and written communication skills. •Ability to lead and work within the interdisciplinary setting. •Strong organizational skills.
Able to manage changing priorities per needs of the PACE program and the agency
SUPERVISORY RESPONSIBILITIES Provides direction and supervision to related primary care, nursing and allied health services.
EDUCATION and/or EXPERIENCE Education: Medical Doctor with emphasis in geriatrics. Board certified in Internal Medicine, Family Practice, or Geriatric practice desired. Current state license and DEA registration required. Meets requirements of 42 CFR Part 410.20.
Experience: A minimum of one year experience working with a frail or elderly population required.
CERTIFICATES, LICENSES, REGISTRATIONS 1.Board certified in Internal Medicine, Family Practice or Geriatrics. 2.Florida state license and DEA registration. 3.Cardio Pulmonary Resuscitation (CPR) certification required.

Requires manual and finger dexterity and eye-hand coordination. Must be able to document care provided in participants’ service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers.
2.Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, caregivers, participants, and community at large.
3.Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.
