Renaissance Social Services, Inc. (RSSI) is an innovative social service agency that provides housing stability services to persons who are homeless and disabled. We are currently looking for a full-time Clinical Case Manager, MHP or QMHP, for our Supportive Housing Outreach Team (SHOT) program.
The SHOT program provides intensive supportive services to homeless program participants with mental health and substance use diagnoses living in scattered site units across the city of Chicago.
Services include mental health case management attending to the psychosocial, health, financial, recreational, nutritional, and housing needs of the clients. Our primary population consists of people with some form of mental illness, such as schizophrenia, bipolar disorder, or depression and a history of drug and alcohol use. In addition, many of our clients have a medical diagnosis.
The goal of the program is to help clients attain permanent housing and achieve greater autonomy, develop disability management skills and increase their social support network. This position entails driving to visit clients located in scattered apartments throughout the City of Chicago and 1 week of on-call every 2 months. Driving of clients in your personal car may be required.
Responsibilities include: Helping homeless clients attain housing, providing intensive outreach based case management, coordinating services with other service providers, transporting clients to medical appointments if necessary, and linkage, referral and advocacy with services in the community, providing service planning, coordination, and implementation of services needed, crises intervention services when needed, working with clients and residents to increase their skills and income, improve their self-determination, and maintain residential stability, maintaining the confidentiality of client information in any form and maintaining accurate and up-to-date records on all clients, completing required Medicaid billing and documentation in a timely manner and completing a minimum number of Medicaid billable hours per week.

The ideal candidate should have a Bachelor's Degree in Social Work, Human Services or Counseling. Seeking someone with a background in case-management and medicaid Rule 132 billing. This position requires an individual that is able to work well independently and be highly organized. Must be computer literate, have a vehicle with current Illinois insurance and have a valid drivers license.
