The Regional Travel Nurse Practitioner will be deployed outside of their home market. Primary Responsibilities:Travel up to 100%; regions are identified and updated based on business needsConduct in-home assessments on health plan membersIdentify diagnoses to be used in care management and active medical managementFormulate a list of current and past medical conditions using clinical knowledge, judgment and current findingsCommunicate gaps in care to member's PCPIdentify referral needsIdentify urgent and emergent situations for proper interventionEducate members on topics related to health conditions and promotionCompliance with all HIPAA regulations and maintain security of protected health information (PHI)The regional traveler role covers multiple states including California, Washington, Oregon, Nevada, Arizona, Utah and Idaho
Required Qualifications: FNP, GNP, ANP, board certified or eligible with an active, unencumbered licensure free any open/unresolved disciplinary actions including probation or restrictions against privilege to practice Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area Proficient with electronic medical records and the internet Ability to lift 30 pound bag in and out of care and up and down stairs as needed Ability to navigate stairs and a variety of dwelling conditions and configurations Ability to sit, stand and kneel as needed to perform physical assessment Strong communication skills with the geriatric or Medicare population Detailed oriented; dependable and reliablePreferred Qualifications: 3+ years of experience in a clinical setting Home Health Care or Nursing Home experienceUnitedHealth Group is working to create the health care system of tomorrow.
