Under the direction of the High Risk Program Leadership, the High Risk Program Care Coordinator works with high-risk patients to help reduce preventable hospital/emergency department, overutilization and/or underutilization of services, promote health maintenance and management of chronic diseases. The High Risk Program Care Coordinator works closely with the High Risk Case Managers, Social Worker, and Pharmacists as key members of the interdisciplinary care team. The High Risk Program Care Coordinator will assist in helping members navigate the complex health system, help schedule appointments, identify and coordinate resources, educate patients on the provider's treatment plan, promote self-efficacy, and identify & resolve any barriers.
Experience working in a health care setting.
Previous Primary Care office experience.
Front and back office experience.
Previous experience working in community health education for vulnerable populations, understanding of social factors that contribute to need for help with health system navigation for wellness.
Excellent communication skills both verbally and in writing.
Knowledge in MS, Excel, PowerPoint and Access.
Demonstrated ability to coordinate and research the review of difficult, complex Utilization Management issues.
Minimum three (3) years of experience in a health care environment.
Knowledge of third party payors.
Electronic Medical Record/EPIC experience.
Must be able to work various hours and locations based on business needs.
