Be a part of building a new home-based clinical program in Tennessee. TeamMD is actively recruiting board certified, community-based, primary care physicians to join our practice. We are an innovative, home-based, physician-led team focused on transforming care for frail, vulnerable, and high-risk individuals with complex needs. The physician will work in conjunction with dedicated teams to address integrated care needs for our patients. In this role, you will provide direct care of chronically ill and high risk patients in a variety of environments including private homes, assisted living facilities, nursing homes, and / or skilled nursing facilities, including assessments of conditions and shot term stabilization and transitional support. In addition, you will supervise and support a team of nurse practitioners to do the same. You will direct and collaborate with a multidisciplinary team to engage resources and strategies to address medical, behavioral, functional, and social barriers to care. Primary Responsibilities: Provide oversight and review for in-home assessments, transition activities, and medical care / services provided by TeamMD staff Review, interpret, and integrate data to guide best practices Provide temporary stabilization services for post-acute care Facilitate clarification of patient and family goals of care to support development of a comprehensive care plan. Provide education and information on health conditions, illness trajectory, and treatment options to patients, families, caregivers, and providers. Provide input to organizational quality initiatives and staff education Lead multidisciplinary interdisciplinary care team (ICT) meetings to improve patient outcomes and satisfaction, as well as optimize clinical integration, efficiency, and effectiveness Mentor other caregivers Drive new initiatives through effective written / verbal communication with staff Inform and collaborate with TeamMD leadership on business and clinical initiatives within the local healthcare market Monitor provider performance and provide consistent coaching and feedback for staff development
Required Qualifications:Active and unrestricted medical license as a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) within the state of Tennessee Board certification in Primary Care-Internal Medicine or Family Practice preferred Current and unrestricted DEA certificate 7 or more years of clinical practice Collaborative interpersonal and leadership style Experience working with patients in non - clinical settings Recognition of the value in team-based care Strong analytical and problem solving skills Ability to think creatively and be comfortable in an innovative and rapid-growth environment Collaborative interpersonal and leadership style Recognition of the value in team-based care Strong analytical and problem solving skills Ability to think creatively Comfort with innovation and rapid-growth environment Experience working with Electronic Medical Records Primary residence in TennesseePreferred Qualifications:3 or more years of medical management or practice leadership Experience in caring for vulnerable populations Knowledge and understanding of the healthcare finance including Medicare and Medicaid Careers at TeamMD: Imagine.
