This position is in the Department of Student Health Services. This position is for the Danforth Campus in the Habif Health and Wellness Center.
The Care Manager will help facilitate student access to and utilization of university and/or community resources, particularly clinical treatment resources. Care management may include meeting with clients who need more intensive outpatient or inpatient mental health services, obtaining external referral services for students, maintaining a database of community providers, and coordination of care including follow up communication and/or meetings with clients or community providers. Though the primary role is care coordination, this staff member may also assist in providing some clinical, consultation, or psychoeducational services on an as-needed basis, time permitting.
PRIMARY DUTIES AND RESPONSIBILITIES:
Collaborates with MHS staff, community providers, and other campus partners in the planning, implementation, and coordination of care of MHS clients.
Maintains an existing database of referral resources and services in the local community, including low-cost providers. Assists in on-campus referral processes as needed and maintains strong relationships with on-campus partners.
Communicates with community providers and agencies to expand and maintain referral networks and relationships. Assists in improving internal and external referral processes.
Tracks students referred to the community and communicates with them as needed about continuity of care (whether they completed the referral process, quality of care from various providers, etc.).
Assists students in the off-campus treatment referral process with guidance on issues such as understanding and evaluating insurance benefits, transportation, selecting a therapist, and knowing what to expect from initial meetings.
Coordinates hospitalization and aftercare planning; serving as a liaison to local hospitals.
Assists with pre-authorization requests for psychiatry medications.
Provides case management services such as brief check-ins with certain high risk students.
Provides consultation to faculty, staff, students, and family members who are concerned about a student.
Perform other duties as assigned.
Master’s degree in a mental health related field or RN with experience in a mental health setting.
Licensed or license eligible in the State of Missouri.
Masters degree in Social Work, licensed or provisionally licensed with 2 years of case management experience OR Registered Nurse licensed in the State of MO with minimum 2 years mental health services case management.
Interest and experience with the developmental and mental health needs of traditional aged college and graduate/professional students.
Significant experience with triage, assessment, and or crisis intervention in a fast-paced, high volume, mental health setting
Excellent interpersonal skills and experience in mental health consultation.
Interact effectively with both health care and non-health care professionals and a diverse population of students.
The hiring range for this position is $46,215 - $60,079 annually.
