Under supervision of the Branch/Area Manager, plans, staffs and supervises day-to-day work activities of the department. Assists in developing strategic plans and department policies and procedures. Implements patient care goals, and monitors adherence to quality assurance, safety and risk management policies. Interviews, hires, trains and assigns work to staff.
Manages patient care for an assigned group of clinicians, directing utilization of resources to ensure effective, efficient, appropriate care. Case management includes review of admissions, assessments, SCIC's, LUPAs, recerts and dis-charges and ongoing monitoring of the plan of care. Completes patient care needs/requirements by scheduling and assigning clinical staff and folowing up on nursing and staff; following up on staff work. Promotes and restores patients' health by developing day-to-day management and long-term planning of the patient care area; directs and develops staff, collaborates with physicians and multidisciplinary professional staff; provides physical and psychological support of patients, families and caregivers.
Provides ongoing education and support to clinicians regarding documentation, case management, resource utilization and quality clinical practice. Holds staff accountable to organizational guidelines, policies and procedures and standards of clinical care.
Provides follow-up and support to other areas regarding authorizations, service batch and incomplete and/or missing documentation. Participates and supports strategic plans including but not limited to: maintaining employee and patient safety, promoting clinical outcomes, meeting productivity requirements, and customer-service standards; resolves patient/customer/stakeholder and employee issues and concerns; identifies improvement opportunities and implements as indicated.
Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; and participating in professional activities.
Assures appropriate levels of service and supplies are utilized to maximize potential for positive clinical outcomes, while managing resources efficiently including making recommendations for use of specialized staff such as ET's dieticians to reach optimal clinical outcomes efficiently including.
Reviews cases to assure that the POC is adhered to and effective patient teaching is done. Performs documentation reviews to ensure plan of care is adhere to including medication reconciliation and utilization of disease management guidelines.
Monitors staff schedules on a daily basis to maintain productivity and quality patient care visits utilizing the interactive scheduler. Triages patients appropriately.
Participates with Branch Manager in hiring, educating, counselling and evaluating field staff, including field observations of clinical staff to ensure quality of care and client satisfaction. Conducts team meetings and staff meetings month. Adheres to organizational guidelines, policies and procedures and standards of clinical care.
Monitors key indicators and department operations, making adjustments and recommendations to the Branch Manager to ensure that goals are met and includes pending patient, case manager, visit timeliness and LUPA reports. Works with clinicians to improve clinical outcomes and patient satisfaction utilizing SHP.
Promotes employee and patient satisfaction with rounding. Enforces staff adherence to clinical standards and practice acts, to state, federal and other governing agency regulations. Performs patient visits as indicated.
Performs other duties as assigned to include communication with physician office regarding orders and lab results and supervisory on-call duties. Making Home Care visits as needed.
Qualifications Minimum Requirements:
Current Missouri RN license
Valid driver's license
CPR certification required.
1 years of experience required as an RN (acute care experience strongly preferred, but not required.)
1 years of Home Health or Hospice RN experience strongly preferred
Requires effective oral and written communication skills to communicate with customers. Requires good decision making and problem solving skills. This position requires occasional travel to patient homes and meeting locations, the ability to operate a vehicle in a safe and lawful manner is an essential function of the job. Additionally, access to a vehicle in good working condition required.
