Supervision of 3 STAR case managers and 6 recovery specialists
Communicating regularly with the DMHAS Star Coordinator
Demonstrating progress toward program goals
Coordinating and monitoring of program services
Collaborating with systems partners to ensure coordination of care
Ensuring that recovery program services are supported by addressing trauma
Ensure services are delivered in a culturally competent and linguistic manner
Improving the scope and capacity of the delivery system in order to ensure program sustainability
Ensure case managers are utilizing a comprehensive case management assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs
Work with case managers to develop an Integrated Family Case Plan that is person-centered and includes strategies for recovery
Outreach to appropriate care and resources in the community including resources that address specialized needs, such as agencies providing services related to HIV/AIDS, mental health disorders, chronic and acute health problems and problems stemming from involvement with the criminal justice system;
Supervise the referral of clients and help them access substance use disorder treatment if needed
Coordinate needed interventions and services provided by multiple agencies;
Oversee supervision of case managers to ensure that individuals are both informed about and able to access the full range of medical and treatment opportunities or other support services, such as legal, financial, educational, vocational, or employment counseling
Perform other duties as assigned
Master's degree in health, psychology, counseling, social work, education or other behavioral health profession.
Demonstrate evidence of working with individuals with substance use disorders and/or evidence of addiction coursework.
Minimum 3 years prior experience, including supervision.
