We are currently seeking a full-time Family Resource Developer to join our Midway Mile Square Health Center Team!
ESSENTIAL JOB FUNCTIONS:
Works collaboratively as a member of a team that includes: families, children, school and agency staff, trauma informed agency and primary care staff, and other providers in the community.
Communicates effectively to diverse staff and clients in both written and verbal formats.
Provides linkage to a wide variety of community resources through effective verbal and written communication.
Conducts outreach, recruitment, engagement and education to diverse families and community members.
Partners with primary care and mental health staff to understand resource needs of families, identifies demographics, conducts needs assessment, identifies appropriate resources and barriers to accessing these.
Provides peer support and mentoring and psychoeducational groups within clinic and Learning and Wellness Center.
Supports parent involvement in children’s social-emotional, cognitive and physical development.
Links parents to educational resource materials to assist them in meeting the academic needs of their child(ren), attends 504 plan and IEP meetings as needed.
Helps parents and caregivers identify gaps, barriers, and issues that interfere with a child’s healthy physical and emotional development and readiness to learn.
Empowers and educates parents on the importance physical and emotional wellness play in early childhood and overall child development.
Helps educate parents on the importance health and nutrition plays in early childhood and overall childhood development.
Has a good understanding of normal childhood development across all age groups.
Advocates to medical and school staff and community agencies to secure needed services, resources and benefits.
Attends meetings and in-service training as required by funder.
Documents services and activities as required by agency, funding sources, and regulatory bodies, including electronically.
Assumes additional responsibilities, tasks and duties to support clients and their families as determined by the Program Supervisor.
Is aware of a variety of community resources and seeks to increase knowledge of and partnerships with new and existing organizations.
Utilizes knowledge and personal experiences to support clients and assist with navigating the mental health, social service and education systems.
Demonstrates flexibility and organization skills within a fast paced medical environment.
Works effectively with diverse staff and service population.
High School diploma or equivalent required.
Must be a parent or family member who has navigated the mental health system with his/her own child or family member.
As required by program, ability to maintain community, office and home-based assignments, valid driver's license, car insurance, and good driving record.
Ability to work effectively with diverse individuals and groups.
Must have the ability to work independently, communicate effectively and work as a part of a team.
