Community Health Partnership of Illinois (CHP) is seeking a Nurse Practitioner to provide comprehensive patient care at our Harvard clinic sites and work with the CHP healthcare team to ensure delivery of high quality, efficient and patient-centric care. CHP is a FQHC with multiple clinic sites in northern and central Illinois providing medical, dental and support services to over 9,600 patients annually. Our administrative offices are located in Chicago.
Key Duties & Responsibilities:
Establishes community collaborative linkages with other community agencies and migrant programs to provide comprehensive services for patients and to avoid duplication of services
Initiates a comprehensive, ongoing identification of patient needs in order to develop and implement a plan of services and advocacy for that patient
Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice
Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues
Valid IL AP Nurse license
IL Nursing license
Federal DEA license
Bi-lingual English/Spanish strongly preferred
Valid Driver's license and automobile insurance and the desire and flexibility to work in multiple locations
