Position Summary: We have an exciting opportunity to join our team as an Assistant Director, Pharmacy.
In this role, the successful candidate will be responsible for daily activities of clinical pharmacy services.
Acts as a liaison to the medical and nursing staff Participates in patient rounds with assigned medical team Precepts Pharmacy Students Responsible for implementation, support of Joint Commission Standards Involved with all aspects of medication management, patient care and clinical support Prepares P&T agenda, minutes Oversees medical staff education Oversees staff education program; provides required educational programs Reports on Medication Use Review data to the P&T Committee Collects data related to medication use reviews Oversees all aspects of medication monitoring
JOB REQUIREMENTS: Minimum Qualifications:
PharmD from an accredited School of Pharmacy required. Post Graduate Year 1 (PGY1) and Post Graduate Year 2 (PGY2) required. 3+ years of hospital based Clinical Pharmacy management experience required. New York State Pharmacist license required.
