Reporting to the Assistant Manager of Internal Medicine and Acute care assumes responsibility for all nursing responsibilities that support a primary care clinical team for Adult Medicine.
Responsibilities include direct nursing care for chronically ill populations, as well healthy populations. Responsibility includes patient centered direct care for acute/episodic needs of member population. Clinical judgment and decision making in accordance with clinical practice guidelines and protocols and clinician orders, including follow up, preventative health, maintenance, teaching and education. Assume responsibility for evaluation and disposition of scheduled patients as well as same day- walk in patients. Significant emphasis on telephone nursing/ triage, as well as case management and care coordination. Will function as a liaison to clinicians regarding triage/clinical care decisions and facilitate patient flow. Anticipate patient care needs based on patient assessment and prioritize care. Foster ongoing improvement of access to services.
Nursing responsibilities will include Visit RN role, Team/Care Coordination RN role, Walk-in Triage Role, Acute Care Direct nursing role, and Telephone Triage RN role. Oversight and supervision of support staff on a care team and the ability to delegate to clinical and support personnel, within their scope of practice/responsibility. Responsible for performance evaluations, training, coaching and orientation of members on a care team. Participate in departmental quality improvement activities. Communicate with all health care clinicians to ensure appropriate on-going care. Proficient in the placement and maintenance of I.V. therapy.

Bachelor's Degree in Nursing and two years' experience or an equivalent combination of education and experience.
