Swedish Medical Group's Residential Care Team is committed to delivering the highest quality care for our frail and vulnerable patients in an interdisciplinary model where nurse practitioners work closely with physicians. Care focuses on post-hospital patients during their rehab stays in our partner transitional care units, as well as primary care for residents and long-stay patients living in retirement communities in both independent and assisted living settings.
