The Health Educator role provides program and administrative support of educational programs in Health Management and Education (HME). This position will work collaboratively with other team members to facilitate learning and assist patients in finding new ways to inspire healthier habits. The health educator will be responsible to engage patients telephonically, electronically and in group education sessions and provide them support and encouragement as they work towards better health. This position will be responsible for engaging in communication and outreach activities, teaching introductory health classes, contributing to educational program planning and development, and providing organizational and administrative support.--Bachelor's Degree in health education, health science, or health administration with concentration in education.--Skills and abilities to teach and facilitate health classes and deliver information to a diverse population including patients with multiple medical issues, needs, and disabilities. --Minimum of 2 years experience teaching classes for patients regarding wellness or disease specific topics.--Demonstrate skill/knowledge to work in a PC networked environment. Demonstrate skill and ability to utilize Microsoft Word, Excel, Access, PowerPoint, and Publisher; Microsoft Outlook, Visio software, Eclipsys and accessing and contributing to the electronic medical record. --Demonstrate knowledge of marketing principles, design and layout skills to produce professional-looking flyers, posters, web pages, etc. for public and health system dissemination. SPECIAL REQUIREMENT--Must be able to travel to main medical campus and Primary Care Network (PCN) clinics located in the greater Sacramento region, work weekends, evenings and occasional overtime to assist with educational programs or meet department commitments. --THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT
