<p>OBJECTIVE/OVERVIEW<br />CUHCC’s mission is transforming care and education to advance health equity. This position directs the continuum of care coordination services provided at CUHCC. The position manages CUHCC’s Health/Behavioral Health Home services, ensuring that CUHCC retains its certification and that all HCH/BHH services are billed to maximum capacity. The Care Coordination Manager will also provide direct patient care which may include therapy. This position manages the Office of Justice grant which includes managing the DV/SA program and supervising the victim advocates.<br /><br />REPORTING RELATIONSHIPS<br />This position will initially report to the Associate Director and will eventually report to the Clinic Manager. Position directly supervises care coordinators (which include social work interns) and DV/SA advocates.<br /><br />ESSENTIAL FUNCTIONS & PERCENTAGE FOR EACH FUNCTION:<br />1. PROGRAM DEVELOPMENT AND IMPLEMENTATION (30%)<br />-Work to ensure that care coordination team improves patient, clinical and financial outcomes.<br />-Work with other CUHCC stakeholders to design and implement a set of metrics for HCH and SAMHSA with annual goals.<br />-Develop an annual workplan for Care Coordination.<br />-Determine optimal/practical staffing and qualifications for care coordination.<br />-Create job descriptions and staff competency expectations.<br />-Write and implement detailed standard work flows for care coordination.<br />-Design a training program for care coordinators to meet care coordination needs.<br />-Analyze and recommend care coordination, education and other software and technology. Implement software as possible.<br />-Determine workloads for care coordinators. Incorporate these expectations into the work flows, hiring expectations and staff evaluations.<br />-Determine annual workplan for DV/SA advocates.<br />-Develop the process to access and select social work interns and the social work triage position. Interview and select interns.<br />-Work with the liaison from the School of Social Work to oversee and monitor the work of the social work interns including the completion of necessary paperwork and onboarding.<br /><br />2. SUPERVISION AND MANAGEMENT (30%)<br />-Hire, assign, and train care coordinators, integration specialists, community health workers, peer specialists and other staff to achieve the goals outlined in the annual care coordination workplan.<br />-Determine and assign patient panels.<br />-Disseminate tasks to care coordinators and ensure that metrics are being met.<br />-Participate in FUHN CQI/Care Coordination meetings.<br />-Working directly with care coordinators and indirectly with other staff, ensure that CUHCC is meeting BHH requirements.<br />-Ensure that CUHCC is optimally billing for BHH work. <br />-Ensure that a maximum amount of patients receive care coordination services.<br />-Monitor care coordination metrics on a daily/weekly/monthly basis.<br />-Define performance standards for direct reports that represent individual and team contributions to business outcomes, cultural competency, and patient and staff satisfaction.<br />-Ensure that care coordinators receive continued training and skill development.<br />-Hold regular meetings (individual and team) with direct reports to ensure staff engagement, performance, and satisfaction. Ensure that meeting minutes are taken and filed.<br />-Review the performance of care coordinators on an ongoing basis. Record conversations, kudos, coaching and other pertinent data into the care coordinator’s supervisory file.<br />-Work to develop a high performing care coordination team that is responsive to patient needs.<br />-Provide supervision to Social Work interns and Social Work Triage position.<br />-Participate in Quality, CLOPs and other meetings.<br />-Develop a monthly dashboard report for care coordination, DV/SA, and social work. <br /><br />3. PROVIDE DIRECT PATIENT CARE (UP TO 30%)<br />-Provide direct therapeutic care and/or care coordination including therapy and diagnostic assessments, to participants in HCH, SAMHSA, and other programs.<br /><br />4. Other 10%<br />-Lead and attend staff meetings.<br />-Participate in quality improvements efforts.<br />-Build community partnerships.<br />-Other duties as assigned.</p><p> </p>
<p>REQUIRED QUALIFICATIONS<br />-BA/BS and six years professional work experience or advanced degree and four years professional work experience, or terminal degree and 1 year experience. Current LICSW license in Minnesota or ability to receive LICSW licensure within 12 months of employment.<br />-Experience and success in staff supervision and team-building.<br />-Training or experience in health care and/or social services for patients with cultural, income, linguistic and other barriers to care.<br />-Strong project management skills, analytical aptitude and problem solving skills.<br />-Strong time management and organizational skills.<br />-Excellent verbal and written communication skills.<br />-High level customer service knowledge and skills.<br />-Proficiency in utilizing an electronic medical record, registries or databases.<br />-Capacity to effectively communicate with staff, stakeholders, and patients in difficult clinical situations.<br />-Ability to motivate patients and team members.<br />-Knowledge and interest in serving patients with mental, medical, dental, sexual, and chemical health needs</p><p>PREFERRED QUALIFICATIONS:<br />-Current independent clinician’s license in mental health field, LICSW<br />-Bilingual and/or bicultural in one of CUHCC’s dominant languages: Somali, Hmong, Spanish, Vietnamese, or Lao<br />-Experience in managing clinical care using an electronic health record<br />-Knowledge of community resources and/or social service agencies<br />-Experience in providing care coordination to a complex, diverse population<br />-Training or experience in quality improvement methods (Lean, Six Sigma, etc.)<br />-Experience working in a primary care clinic or FQHC.</p><p> </p>
