PURPOSE/MISSION: The Intermountain Healthcare PGY2 Ambulatory Care Pharmacy Residency will develop clinical pharmacy specialists in a variety of ambulatory care settings equipping them with the leadership skills necessary to develop a patient centered pharmacy practice in collaboration with the healthcare team while also supporting the profession through education and research. To achieve this, residents will 1) Establish a patient-centered, evidence-based, collaborative interdisciplinary ambulatory practice in order to provide treatment for chronic and acute illnesses in all degrees of complexity. 2) Develop leadership skills required to further the pharmacy profession through advocacy, education, public health promotion and research. 3) Utilize effective communication skills to develop professional relationships and become an integral member of the healthcare team. 4) Demonstrate the skills necessary to become an effective educator of healthcare professionals and healthcare professionals in training. 5) Serve as an authoritative resource on the optimal use of medications in the ambulatory setting.
PRACTICE SITES/ROTATIONS: Residents will train in a variety of ambulatory care clinic sites across Intermountain Healthcare’s system of hospitals and clinics. They will benefit in many ways from completing specialty training within a nationally-recognized integrated healthcare system. Primary rotation sites will exist at Intermountain Medical Center, McKay-Dee Hospital, Primary Children’s Hospital, and Intermountain Medical Group Clinics. Residents will have a core continuity clinic rotation in family medicine or internal medicine. Additional required rotations include geriatrics and practice management. Elective rotations in transplant, heart failure, psychiatry, pediatrics, pain management, diabetes, academia, oncology, and others are also offered. Residents will have opportunity to precept students and PGY1 residents and lecture at one of two local colleges of pharmacy. A teaching certificate is offered. A service development/enhancement project and a research project of publishable/presentable quality are required. Limited staffing and on-call (at home) requirements are expected, which is consistent with all Intermountain Healthcare residents.
Doctor of Pharmacy Degree (ACPE accredited), PGY1 residency, pharmacist licensure in Utah, willing to complete an onsite interview
