Responsible for case management, advocacy, and individual counseling to both youth and young adult clients in the Community Violence Intervention and Prevention Program
Conducts intakes and assessments and timely individualized service plans for each participant which includes specific and measurable short and long term goals and objectives. Monitors client’s progress through routine office and home visits with all clients.
Works independently and consistently exercises discretion and sound judgment in providing supportive and crisis counseling to clients.
Maintains up-to date and accurate client’s files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies.
Knowledgeable about community resources and links clients to appropriate resources. Advocates on behalf of clients in order to ensure access to services for which the client is eligible.
Coordinates client services with the program’s therapists in order to help ensure the client is able to meet short and long term goals related to individual therapy.
Assists with program’s Performance and Quality Improvement activities.
Solicits, logs, sorts and transports and places in storage in-kind donations for the program. Delivers needed basic household items and other resources to client’s home when necessary.
Participates in community meetings and represents the agency regarding services provided as well as providing accurate and timely reporting to the Clinical Manager of the meeting discussions.
Some evening and weekend hours required to meet client needs.
Hours: Full time 4 positions (2 positions require candidate to be Bilingual)
Work Location: 2 positions located at 5645 W. Lake Street, Chicago, IL. 60644 and 2 positions located at 671 S. Lewis Avenue, Waukegan, IL. 60085
Preferred Requirements: MSW. Three years of relevant experience. LCSW or LCPC. Bilingual
Minimum Requirements: Master’s degree in related field. Two years of relevant experience
Other Requirements: Driver’s license and reliable transportation.
