Rutgers, The State University of New Jersey is seeking a Senior Care Coordination Specialist for the Environmental and Occupational Health Sciences Institute. The primary purpose of the Senior Coordination Specialist position is to skillfully assess, advocate, plan, implement, coordinate, monitor, and evaluate health care options and services to meet needs of the patient in the medical neighborhood in accordance with professional case management standards. Among the key duties of this position are the following:
Performs case management and associated administrative duties in compliance with the contractual requirements of a federal health program.
Addresses the medical, nursing and/or personal care needs of the patient to enable their return to an optimal level of functioning.
Conducts a comprehensive needs and risk assessment of the patient (and family/caregiver with consent) to prioritize needs and resource allocation.
Identifies support systems that indicate actual or potential problems and sets health care goals based on needs and risk assessment findings.
Re-assesses the member's progress towards identified care goals and collaborates with the multidisciplinary health care team to adjust the patient's care plan, as medically appropriate.
Ensures system/process is in place to deal with emergency/high-risk patients.
Identifies cost-effective resources while providing quality care to reach the desired outcome.
Follows up with patients when certified of conditions covered in the federal health program to identify needed services and coordinate care accordingly.
Ensures referrals are coordinated and service summary reports are maintained in the electronic health record (EHR) system.
Assesses pre-and post-authorization compliance and assists in auditing activities to ensure medical records are complete and in accordance with federal standards.
Communicates with the patient's Patient-Centered Medical Home provider to ensure patient receives care for conditions not covered in the federal health program.
Maintains medical records in the EHR system in accordance with established guidelines.
Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
Performs other nursing function duties as assigned.
Minimum Education and Experience:
Bachelor's degree in Nursing or a health care related field.
Registered Nurse with a license to practice in the State of New Jersey.
At least two years of case management experience.
Required Knowledge, Skills, and Abilities:
Must have knowledge of Microsoft Office Word, Access, Excel and Outlook.
