The Center for Innovative Pharmacy Solutions at the University of Maryland, Baltimore School of Pharmacy is hiring for three (3) Advanced Practice Pharmacists. The Center will be launching programs supporting the practice model goals of improving interprofessional care coordination and medication therapy management for adult populations with chronic diseases. The care delivery model involves clinical and process of care training; care coordination; telehealth services; and quality assurance overview. Through care coordination and shared access to electronic medical records, the project will enable pharmacists to participate in improving medication adherence and management, resulting in better health, reduced hospitalizations and emergency room visits, and fewer adverse drug events for patients with multiple chronic diseases.

Help develop the process of care for pharmacists in the program; finalize protocols; establish policy and procedures.
Assist with pharmacists and pharmacy technician training activities.
Provide clinical services that support day-to-day operations and safe practices.
Provide oversight and programmatic support to providers.
Perform Q&A activities to assure process of care consistency and effectiveness.
Work with Project Team to participate in strategic planning and innovation to meet institutional, departmental, patient care, and regulatory needs.
Engage with the team to support financial stability and growth of this innovative practice model.
Participate in processes for monitoring and analyzing performance by provider and monitor revenue generation.
Develop, review, and maintain medication and chronic disease state management plans related to process of care requirements.
Mentoring and supervising residents and students.
Education: Bachelor degree in pharmacy required. Preferred Education: Doctoral degree in pharmacy (PharmD). Licensure: Current State of Maryland Pharmacist license or current licensure reciprocal with the State of Maryland Board of Pharmacy (able to secure and retain an active State of Maryland Pharmacist license for employment). Licensure must be in Good Standing. Experience: One (1) year post baccalaureate or post doctoral residency plus one (1) year of experience
Two (2) years post baccalaureate or post doctoral residency;
Certification in a relevant specialty by the Board of Pharmaceutical Specialties or Commission for Certification in Geriatric Pharmacy Preferred Experience: Experience with delivering telehealth services or healthcare call center operations preferred.
Knowledge, Skills, and Abilities:
Knowledge of best practices for medication therapy management and clinical services.
Skill in the development of execution of self-management services for chronic diseases.
Knowledge of relevant accreditation and certification requirements.
Ability to establish goals, structures, and processes necessary to implement a patient care program.
Ability to prioritize ongoing and new projects, as well as conduct research and gather information.
Strategic and analytical thinking skills with an ability to solve problems and make decisions.
Oral/written communication, presentation, and interpersonal skills
