Sinai Urban Health Institute (SUHI) is hiring a bilingual (English/Spanish) Community Health Worker (CHW) to work across SUHI’s health interventions related to breast health, asthma and diabetes. Under the direction of the Supervisor of Program Initiatives, the CHW will be responsible for providing and coordinating health education, as well as case management services. The CHW will serve as a bridge between the patient and the medical system by building trusting relationships with community members served by the program. Specifically, the CHW will be responsible for community outreach and providing health education in a variety of settings including health fairs, community events, group sessions, and other formats as needed. The CHW will also help in identifying barriers to accessing quality care, work with individuals to overcome these barriers, and provide referrals. For asthma, the CHW will work closely with program participants in their homes to provide comprehensive asthma and healthy homes education. In the area of breast health, the CHW will work with women to obtain timely and routine breast health screenings, and assist women with navigating the health care system to obtain screening results and appropriate diagnostic and treatment services as needed. For diabetes, the CHW will help clients create personalized wellness plans and promote diabetes self-management. Activities will also include program promotion, enrollment and collecting health information from those interested in SUHI’s programs. SUHI is seeking an enthusiastic, passionate CHW candidate that meets the job requirements outlined below.
Position Requirements: • Must have high school diploma or GED • Previous experience in health education preferred, but not required • Knowledge of Chicago’s Westside and/or Southside communities (resident of Westside or Southside communities preferred) • Passion for and dedication to working with community members and providers • Punctual, reliable and willing to learn • Must have a valid driver’s license and own a car • Proficiency in Microsoft Word, Excel, Access and PowerPoint preferred
