Provides education, appropriate referrals and services to caseload of 60 Midwest Healthy Start at-risk families.
Helps at-risk families take all steps needed for safe delivery and healthy babies.
Assesses the educational, medical and social needs of Healthy Start clients who are at-risk for poor pregnancy outcomes.
Conducts home visits, well-women visits, post-partum visits, and provides health support to individual children and families, as needed.
Initiates provider tracking for clients to ensure clients’ access to medical care and referrals for services.
Works with Midwest Healthy Start staff and provides technical assistance, including proper procedures for the prevention of the spread of illness, assessment of safety concerns and hazards and other topics as needed.
Assists and supports Healthy Start screens from Planned Parenthood and collaborating partners.
Completes initial contacts according to Healthy Start Standards and Guidelines and core performance measures, as required.
Maintains/updates client files and records to ensure Healthy Start, DAP format notes and ETO databases are updated as required per policy (within 24 hours) and Healthy Start Standards and Guidelines.
