Essential Functions: Provide HIV educational sessions to community agencies, schools, clinical sites, as well as to testing and counseling sitesAssist with Outreach to HIV testing and counseling sites to familiarize them with our program and to provide clinical support to HIV identified youth (age 13-24) to facilitate access to medical care, case management and community agenciesProvide hands on support in accessing resources and services, including, but not limited to completing paperwork to obtain insurance, housing assistance, dental appointments, or SSI. Accompany youth to department of Human Services (to get Medicaid, food stamps, etc), other medical appointments Assist with providing HIV testing and counseling to at-risk adolescents and young adultsAssist with clinical/peer support and mentoring to HIV positive teens and young adults within the clinical sites, educational sessions and HIV peer support groupsAssist with filing; record keeping and collecting data and evaluations for the HIV ProgramOther related duties as assigned
