The Infant Clinical Social Worker will be responsible for:
Providing developmental and medical follow-up of high risk infants born prematurely.
Providing psychosocial assessment, supportive counseling and advocacy for families.
Complex care/resource coordination.
Psychosocial consultation to medical team.
Creative contribution to program development.
Outreach to and collaboration with referring hospitals, providers, and community resources such as Early Intervention.
To qualify, you must have:
A Master's degree in Social Work, LICSW required.
2 years minimum of social work experience.
Experience with high risk pregnancy, medically complex infants and children, developmental disability, special education, early intervention and issues around loss preferred.
Flexibility and the ability to work in fast-paced, dynamic environment.
Bilingual (Spanish/English) a plus.
