Provide skilled home care nursing services during intermittent visits to multiple patients on a daily basis. Assess physical, psychosocial and environmental needs of patients and families in accordance with agency, state and federal regulations. Coordinate care for assigned caseload of patients and families as a member of a multi-disciplinary home health team. Document appropriately via Point of Care computer documentation.

Current licensure to practice nursing in the State of Michigan
Two (2) or more years of acute adult/medical/surgical and/or home care experience in the last 5 years
Ability to demonstrate excellent problem solving and triage skills
Ability to work independently in a multidisciplinary, fast-paced environment as demonstrated by interview, professional references and performance evaluations
Direct experience with all types of vascular access devices, wound care, and oxygen therapies
In-depth experience in patient education (i.e. self-care, disease management state)
Demonstrated proficiency with computer skills, Microsoft Word, e-mail, and smart phones
Excellent communication and customer service skills
Ability to travel from point A to point B throughout the workday as assigned

Home care experience as a Registered Nurse within the last two years
